Boarding Form

I certify that I am the owner of the above animal/animals and I do hereby authorize Riverside Animal Hospital and its staff to board my pet and to administer vaccinations, medications, tests, surgical procedures, anesthetics or treatments that the doctor deems necessary for the health and safety of the above pet(s) while under their care and supervision. If my animal has fleas, ticks, worms or mites, if my pet should injure itself in an attempt to escape, refuse food, or become ill while being boarded, I understand that he/she will be treated. I also understand that I am responsible for payment in full at the time that my pet is being discharged. If I neglect to pick up my pet on the expected discharge date and I am unable to be reached by phone, I understand that I will receive written notice mailed to the above address that my pet is ready to go home. If I do not respond within five days of the written notice, you may assume that my pet is abandoned. You are then authorized to dispose of my pet as you see fit (likely via adoption). Abandonment does not release me from my obligation to pay my bill.
